This modular course is aimed at supporting Healthcare Professionals gain an awareness and  help meet the needs of individuals living with varying degrees of frailty, to deliver improved  health outcome.  All modules will consist of case based presentations


Part One


Module 1 - Introduction to Frailty

  • Understand the construct of frailty
  • Understand the operationalised models used in practice
  • Understand why frailty is important and its relevance to outcomes
  • Demonstrate the use of frailty assessments in a community setting
  • Understanding frailty syndromes and described their relevance to outcomes


Module 2 - Delirium

  • Understand definitions of delirium
  • Demonstrate different assessment used to detect delirium
  • Recognise the important prevention interventions
  • Understand management and treatment of delirium


Module 3 - Mobility and Falls


  • Understand the importance and relevance of falls in older people
  • Demonstrate understanding of prevention and assessment of falls risk
  • Identify the evidence-based interventions to improve mobility and prevent falls.


Part Two


Module 4 - Nutrition and Hydration

  • Understand the importance and relevance of risks associated with inadequate
    hydration and nutrition including AKI
  • Use available tools to demonstrate accurate assessment of nutrition and hydration
  • Demonstrate understanding of early intervention strategies to improve hydration and nutrition


Module 5 - Advance Care Planning

  • Understand different terms and what is an advanced care plan
  • Discuss who and when and advance care plan is used
  • Develop confidence and skills to introduce advance care plan discussions


Module 6 - Polypharmacy

  • Frailty and the Risk of Polypharmacy in the Older Person
