Whether an HCA can administer the first B12 loading dose is not determined by the dose number, but by the prescriber’s clinical judgement, the scope of the PSD, and the competency of the HCA.

There is no national rule stating that nurses or doctors must give the first injection. In theory, an HCA may administer any B12 loading dose — including the first — provided that:

  • A valid PSD authorises administration by a trained and competent HCA
  • The HCA has demonstrated competency in IM injection technique, medicines administration, documentation, and recognition of adverse reactions
  • A prescriber has completed the assessment, diagnosis, and consent before issuing the PSD

Any requirement for a nurse or doctor to give the first dose is therefore a local policy decision, not a legal or national standard.

Prescribers may choose to give the first dose themselves when additional clinical review is needed, for example:

  • Uncertain diagnosis
  • Additional symptoms requiring assessment
  • Concerns about absorption or neurological involvement
  • Desire to assess tolerance or response before continuing

If no extra monitoring is required and the PSD permits HCA administration, the first loading dose does not need to be given by a registered clinician purely because it is the first dose.

  • A clinician must complete the assessment and prescribing
  • Administration may be delegated to a trained, competent HCA under a PSD
  • Any restriction on who gives the first dose is local governance, not national policy
  • The risk assessment sits with the prescriber, not the person administering the injection

Once the prescriber is satisfied that diagnosis, treatment and monitoring are appropriate, the first dose is no different from subsequent doses in terms of administration.
